A THANK YOU LETTER FROM GENTIVA HOSPICE FOUNDATION
January 3, 2012
Dear Foundation Supporters,
Thank you for your recent contribution of $300 to the Gentiva Hospice Foundation, formerly the Odyssey
VistaCare Hospice Foundation, in memory of Joyce M. Johnson.
Our Foundation, which is the non-profit organization established to receive contributions such as yours, focuses primarily on supporting end-of-life causes. For example, we funded pilot programs sponsored by the National Hospice and Palliative Care Organization to increase the public's understanding of hospice and the Medicare hospice benefit and to establish a quality standard for ALL hospices across the country to help assure good care for patients; we provide funds to the Dream Foundation, which is an adult "make-a-wish" organization for the terminally ill, to develop a program to serve terminally ill adults over the age of 65; and we sponsor several bereavement camps for children who have recently lost a loved one. The Foundation also grants assistance for special needs to our patients and families by providing limited aid for those experiencing a financial hardship. In addition, funding is provided to physician palliative care fellowship programs in several major hospitals and many end-of-life programs.
I assure you that through your generosity and the generosity of others, we will continue to support deserving causes associated with hospice care and improving the end of life's jouney. Thank you again.
Sincerely,
Sharon E. Anderson
Executive Director
